Halweg is President/CEO of the Tile Institute of America, TIA. He has earned the recognition of being an internationally acclaimed master expert witness and consultant. His job site inspections, consultations and research for the tile industry number in excess of 200 projects per year. His extensive experience, gathered over the years, has been used in mediations, arbitration, and court cases from small dollar amounts to the largest in the nation. He was the Executive Director of the Ceramic Tile Institute (CTI) for the last 14 years, headquartered in Los Angeles for 40 years prior to its closing late in 1992. He also served as the Chief Executive Officer of Ceramic Tile Institute of America (CTIOA), which also ceased business late in 1992 after 32 years. These were two of the most progressive and dynamic tile associations and Internationally recognized not-for-profit organizations developed to promote tile through quality workmanship and materials until 1992 under the 14-year leadership of Halweg. Gerald M. Halweg has a ceramic background dating back to his academic college courses in 1964. Halweg left two very successful businesses to attend California Lutheran University, from where he honored in Ceramics and received a Bachelor of Arts degree in 1966, plus multiple minors in five disciplines of study. He received a Master of Arts in Education in 1973 from California Lutheran University. He has credentials to teach in nearly every educational level from the State of California. Halweg has earned the Ceramic Tile Consultant (CTC) degree and has been individually tutored for over 10 years by the most highly respected tile mentors in the world. Gerald M. Honored Member of Who's Who™ in Leading American Executives in 1991. Awarded in 1998 and 1999 as an Honored Member of Continental Who's Who™ of Professionals and Executives. Service to Alma Mater Award in 1986. Halweg has a consistent record of success in his every endeavor. Co-Founder and Past President of the Board of Directors, Lutheran High School of Ventura County; Past Chairman of the Board of Education, Ascension Lutheran Day School and Board Member of Marriage Encounter (plus other civic/ educational activities). Charter Member and trustee of the Republican Presidential Task Force. A strong Christian and family man, Jerry and his wife of 34 years, Judy, reside in Thousand Oaks, California. Their three daughters, Gina Joanne Alexander (married to U.S. Navy Pilot Mike Alexander), Disa Dawn also a graduate of CLU, Holly-Anne (honor student) and our son Garrett Milo, pilot and Doctor of Medicine at Loma Linda University. Professional Achievements: He is a professional member and has served two terms as President of the Los Angeles Chapter of the Construction Specifications Institute (an organization with 19,000+ international members). Since joining CTI, CTIOA, and TIA his list of achievements are lengthy and impressive. He spearheaded, invented and developed the first 32 sound rated tile floor systems in the world. He has been the Chief Administrator of the Ceramic Tile Institute's Annual Seminar and Tile Exhibition for many successful years, Director on the Board for Western States Ceramic Tile Association (WSCTA). He is the only tile industry representative, in the U.S.A., with perennial monitoring, writing and proposing code changes for the good of the construction industry and the public. Halweg was instrumental in the coordination of groups throughout California and the writing of the examinations for the California C-54 Tile Contractor's licenses and introduced/ assisted in the formulation of the Tile Contractors' License Programs and Tile Contractors' examination for various other states. Gerald has authorized various standards for the tile industry, including slip-resistant test procedure and others. He has also written numerous articles published in industry periodicals, national magazines, professional documents and test books. Mr. Halweg, for many years was the publisher and editor of Tile Industry News with a circulation of over 50,000 copies bi-monthly in 50 states and 22 countries. OFFICE TIME for plan checking, specification review, writing, research and lecturing according to TIA's schedule. Principal - $100.oo per hour, (to the next quarter hour) Director - $75.oo per hour, (to the next quarter hour) Support Staff - Reasonable rates depending on task. 2. TELE-COMMUNICATIONS; technical advising and the above items upon call or on client's time schedule. Principal - $125.oo per hour, (to the next quarter hour) Director - $100.oo per hour, (to the next quarter hour). 3. TRAVEL time based from portal to portal, starting from Thousand Oaks, California or a location closer to client when two clients can share travel time (arranged by TIA). Principal - $100.oo per hour, (Daily rate may be available for long trips) Director - $75.oo per hour, (to the next quarter hour). 4. TRANSPORTATION ground mileage, portal to portal, at $0.40 per mile. Coach airfare plus mid-size car rental when needed. 5. REIMBURSABLE EXPENSES - Parking, reproductions, film, developing, printing, postage, clerical, special handling of documents, meals and lodging when overnight stay is required. $... (economy to mid-range prices when available.) 6. Inspection, consultation, site examination, failure analysis, tile problem investigation, attendance at meetings and supervision. Principal - $150.oo per hour, (to the next quarter hour). Director - $100.oo per hour, (to the next quarter hour). 7. Mediation, expert witnessing, arbitration, expert exchange meetings, deposition, settlement conferences, trial testimony and standby time for any of the services listed above. Principal - $240.oo per hour, (to the next quarter hour). Schedule of Fees 1998 Page lof2 8. TESTING is in addition to any of the pertinent charges listed above. Each test is priced separately which depends upon the type of test needed and the independent laboratory most qualified and available to perform the test. We perform most tests in house and the balance we contract with our engineers according to their expertise and availability. We test according to ASTM, ANSI, FHA, BIS, TIA, CTI, ICBO-ES, IAMPO, City of L.A. and others depending upon the requirements for the intended need. For an exact quote, a TILE INSTITUTE of AMERICA representative will need to consult with the client as to their needs, time schedules, and if compliance to a particular standard, license or code is necessary. $... (Competitive with quality service, emergency, expediting services available at a slight increase.) 9. A retainer of ($ 500.00) which will be credited to the amount due at progress or final billing. If the amount paid exceeds the amount due at final billing, the difference will be refunded to the payer. The progress and final invoices are due upon receipt. 10. TIME LOST BECAUSE OF CANCELLATIONS OR CHANGES OF SCHEDULES CAUSED BY THE CLIENT OR OPPOSITION MAY BE INVOICED AT THE ABOVE STATED RATES. ANY INVOICE NOT PAID WITHIN 30 DAYS IS SUBJECT TO THE MAXIMUM AMOUNT OF INTEREST PROVIDED BY LAW. This fee schedule will be adjusted annually on December 31.
